Payment is due at the time of your appointment. We accept cash, personal check, money order, and credit and debit cards (including FSA & HSA cards).
Returned checks will incur an additional charge.
We are able to accept most commercial insurance plans, and can provide you with a receipt that you may submit to your insurance company in an attempt to obtain reimbursement for “out-of-network” services.
Please contact your insurance company ahead of time to determine whether or not we are in network, or if not, if you have out-of-network benefits. Please be aware that the reimbursement rate is typically lower when you use out-of-network providers, and will be based on the insurance company’s pre-determined fee schedule, not on our rates.
Please also be advised that reimbursement from your insurance company will be related to your diagnosis and the type of treatment provided. Even if you have out-of-network benefits, certain diagnoses and/or treatment combinations may not be approved for coverage by your insurer.
